The Mississippi County Health Department is accepting resumes for a Community Health Worker (CHW)/ Program Coordinator focusing on chronic disease management. Responsibilities will include advocate and assist community members within the region in getting the care and services needed, provide education to help improve health outcomes, facilitate linkages to services and programs, collect data and complete reports, facilitate the start of Baby and Me Tobacco Free and Freedom from Smoking as well as become a facilitator for both programs, market these services to community members and organizations. Two years of experience in the field of public health, medical care, social services or closely related fields, graduation from a two- or four-year college or university with specialization in healthcare, nursing, education, social services or closely related areas. Excel/Jotforms or other data collection platforms experience preferred.
